Clear, Simple Membership Pricing

Simple, transparent pricing for individuals, couples, and families. Membership provides direct access to Dr. Ford and comprehensive primary care without the limitations of a traditional insurance-based practice.

*Each additional family member beyond 6 is $84/member/month.

A one-time registration fee of $75 per person applies, capped at $300 per family. All memberships require an initial three-month commitment. After the initial commitment period, memberships may be canceled at any time. A re-enrollment fee will apply if you choose to restart your membership after cancellation.

  • Two 30–45-minute follow-up sessions with OMT and metabolic or dietary adjustments

Program Expiration: All sessions must be completed within 1 calendar year (12 months) from the date of purchase. Unused sessions expire after one year.

Important Care Note: This specialty program does not replace primary medical care. Patients must maintain an established Primary Care Physician (PCP) for routine care, medical diagnoses, prescription management, and overall medical management.

  • What is Direct Primary Care?

    Direct Primary Care (DPC) is a membership-based healthcare model that creates a direct relationship between you and your physician without insurance billing in the middle. Members pay a straightforward monthly fee for personalized, accessible primary care and direct access to Dr. Ford.

  • How does FORD NY DPC membership work?

    After submitting a membership interest form, a member of our team will reach out to walk you through the enrollment process, answer your questions, and get you scheduled to begin care with Dr. Ford.

  • What is included in membership?

    Membership includes extended visits, direct access to Dr. Ford, same-day and next-day appointment availability, and comprehensive primary care without the constraints of a traditional insurance-based practice. Reach out for full details on current inclusions.

  • Does FORD NY DPC accept or bill insurance?

    No. FORD NY DPC does not bill health insurance for membership or covered primary care services. Insurance may still be used for services outside the practice, including specialist care, hospital services, imaging, medications, and other services not included in membership.

  • Do I still need health insurance?

    Yes, maintaining coverage for unexpected or higher-cost medical needs is recommended. Direct Primary Care is not health insurance and does not replace coverage for emergencies, hospitalization, surgery, specialist care, or other services outside the practice.

  • Can Medicare or Medicaid patients join FORD NY DPC?

    At this time, FORD NY DPC is not accepting Medicare-enrolled or Medicaid-enrolled patients while the practice's Medicare roster and required compliance processes are being established. FORD NY plans to offer memberships for eligible Medicare patients in the future once the necessary enrollment and compliance requirements are complete.

    Patients age 65+ or enrolled in Medicaid who are interested are encouraged to reach out to be notified when enrollment becomes available.

  • Can families join?

    Yes. FORD NY DPC offers family membership options for couples and families of any size. Reach out via the interest form to learn about family enrollment and pricing.

  • What is the membership commitment and cancellation policy?

    Membership begins with a three-month commitment. After that, members may cancel at any time. Patients who cancel and later choose to return to the practice will be subject to the applicable re-enrollment fee.

  • Can I get a same-day or next-day appointment?

    Yes. One of the benefits of membership is greater access to same-day and next-day appointments, since Dr. Ford maintains a smaller patient panel than a traditional practice.

  • Do I need to schedule ahead, or can I walk in?

    Appointments are scheduled rather than walk-in, which helps ensure members receive dedicated, unhurried time with Dr. Ford. Members can typically schedule quickly thanks to the practice's smaller size.

  • What happens if I need to see a specialist?

    Dr. Ford will refer members to trusted specialists when needed and can help coordinate that care. Specialist visits are billed separately and are typically covered by health insurance.

  • What happens if I need emergency or hospital care?

    Emergency and hospital care are not provided directly through membership. Members should go to the nearest emergency department or call 911 for urgent or life-threatening situations. Dr. Ford can help coordinate follow-up care afterward.

  • How do labs, imaging, and prescriptions work?

    Dr. Ford can order labs, imaging, and prescriptions as part of your care. These outside services are billed separately, often at reduced self-pay or wholesale rates, and insurance may also be used where applicable.

  • Does FORD NY DPC offer home visits?

    Yes. Home visits may be available for members in certain special circumstances, including:

    • Newborn visits during the first two weeks of life
    • Follow-up within seven days of hospital discharge
    • End-of-life care
    • Palliative care

    Other home visit requests may be considered based on geography, medical circumstances, and Dr. Ford's availability. A $150 additional home visit fee applies.

  • What should I do in a medical emergency?

    Always call 911 or go to the nearest emergency department for a medical emergency. Membership is designed for primary care and does not replace emergency medical services.

  • Are employer and community-sponsored memberships available?

    Yes. FORD NY DPC is developing partnership programs for employers and community organizations in the Capital Region. Contact us to explore options.
